COVID-19 vaccine walk-in clinic locations for children

Clinics offering vaccines to children ages 5 to 11 are noted with this icon and subject to change.

All clinics will be closed for Thanksgiving (Thursday, November 25).

Many OhioHealth Primary Care locations now provide COVID-19 vaccines through office visits. Call your provider for more details. You can also visit the Ohio Department of Health website to find retail clinics and other providers throughout Ohio that have the COVID-19 vaccine.

Vaccine boosters

Boosters of the Pfizer, Moderna and Johnson & Johnson vaccines are now available to everyone age 18 and older. OhioHealth will be administering the Pfizer booster at all vaccine clinic locations. Limited doses of the Johnson & Johnson booster vaccine are also available.

How soon after your vaccine can you receive a booster?

  • Six months after your last dose of the Pfizer or Moderna vaccines.
  • Two months after your Johnson & Johnson vaccine.

Which booster should you get?

You can choose a different booster from the vaccine you originally received, as long as you have waited the recommended amount of time from your vaccine.

From the CDC: Individuals may choose which vaccine they receive as a booster dose. Some people may have a preference for the vaccine type that they originally received, and others may prefer to get a different booster. CDC's recommendations now allow for this type of mix-and-match dosing for booster shots.

Third vaccine doses for immunocompromised patients

Certain immunocompromised patients who are more than 28 days past their second dose of the Pfizer or Moderna COVID-19 vaccine are eligible to receive a third dose of vaccine.

  • Talk with your primary care provider to make sure a third dose of vaccine is recommended for your condition.
  • OhioHealth is offering third doses of the Pfizer vaccine, but does not have the Moderna vaccine.
  • Please bring your Pfizer vaccination card or proof of vaccination.
  • Additional doses of the Johnson & Johnson vaccine are not currently recommended.
  • Eligibility criteria, vaccine location search, and frequently asked questions are available on the CDC website.

Access your COVID-19 immunization record or test results in MyChart

OhioHealth cannot replace physical immunization cards, but if you received your COVID-19 vaccine or test at an OhioHealth clinic or location, you can view your immunization record or test results in MyChart.

  • Sign in to MyChart online or in the OhioHealth mobile app.
  • Tap or click Menu, and select COVID-19 in theMy Record section. You have a few options when sharing your vaccination or test result:
    • Option 1) Expand the details of your vaccination or test and take a screenshot.
    • Option 2) Tap or click the Download/Export button and select Download vaccination PDF,or Export to Health Walletif you have a supported app.
    • Option 3) Tap or click the QR Codes button to be scanned at participating locations using the SMART Health Card Verifier app.

If you don't have a MyChart account, but received your vaccination from OhioHealth, you can request an activation code today.

Optional: Request proxy MyChart access for a loved one

If you have an OhioHealth MyChart account, you can request access to a loved one's account to help them with scheduling and managing their care.

In your loved one's MyChart account:

  • Open the menu from the home screen and selectShare My Record.
  • SelectFriends and Family Access, thenInvite Someone.
  • Complete the name and email fields, agree to the terms and conditions, and selectSend Invite.
  • You will have 24 hours to accept the invitation by logging in to your own MyChart account.
